If this is your first booking with Tan Lash & Beauty and you booked over the phone or did not go through the online booking process, we need to direct you to the information below so that we can establish your informed consent. The booking policy applies to everyone, however you need only review the Consent information for the services you have booked. Your attendance at your appointment is indicative of having reviewed this information and accepted the terms.

Booking Policy

REFUNDABLE DEPOSIT: We require a fully refundable base deposit of $50 for services 30 mins+ booked in advance. This does not apply to any spray tan appointments. If you have booked in advance and not paid a deposit inadvertently, we will follow up with your prior to your appointment. If you do not respond and fail to attach a valid deposit your appointment may be automatically released by the online booking system.

AFTERPAY BOOKINGS: If you intend to use Afterpay for your services and would like to do these same for your deposit payment visit to do so in advance https://tlashb.co/afterpaydeposit
The remainder of your booking fee is payable at your appointment as a seperate Afterpay transaction.


CANCEL ONLINE: you can cancel or reschedule your own booking online up to 24 hours ahead via our booking portal! After this cancellations or changes must be made via phone (Press 1 when prompted and our reception will assist).


NOTICE PERIOD: No cancellation fee applies if 24hrs+ notice is given ( 48hrs for bookings 2 hours+) 
Regardless of how close to your appointment / last minute you let us know you can’t make it, in the first instance we will do our best to fill your appointment by making it available to re-book online asap and contacting our waiting list to offer those on it your spot . If we can fill it, we will be delighted to transfer your deposit over to your next appointment. This is why we insist on phone cancellation in the final 24 hours so we can be sure its actioned and try our best to facilitate.

As Lash appointments run on average 75-90 minutes, if we cannot fill your appointment then cancellations within 24 hours forfeit the $50 deposit. This fee does not cover the costs we incur but helps us to remain viable and keep our prices within range for all of our clients, 99% of whom are wonderful and diligent.

AFTERCARE & CUSTOMER SERVICE:

Lashes are a significant investment! We appreciate that our customers are not only trusting us to perform an intricate procedure right next to their eyes but they are also investing a significant amount of money in the result. Our Lash Artists all have Australian training and largely unparalleled experience . We use premium products and particularly invest only in lash adhesive purchased from an Australian company, approved for use in Australia and amongst the most expensive on the market. We go to great lengths to ensure the conditions for Lashing are just right in our salons and almost 100% of our customers are delighted with their results.

At a maximum and usually surrounding the change in seasons, we might encounter 1 client a month who experiences less than perfect retention on their lashes. For Lash Artists operating at the frequency ours do, this number equates to 0.2% of clientele. The science of medical grade adhesive, human hair and the impact of even regular human behaviours in the days following application can be unpredictable and makes pin pointing the cause of these isolated incidents with absolute certainty difficult.

However what we can assure you of is that in the very remote possibility that you have this experience, we will resolve the issue for you and return your lashes to a full set without charge and regardless of fault. We simply ask you contact us within 5 days, several days longer than when any such issue would be evident, and we will make a time for you to come back in at your convenience and no cost .

Although we match your investment in our service with the time of our expert staff and products, we would never accept a rare experience like this as satisfactory for our customers. With such a low occurrence we are more than happy to wear these costs so you don’t have too. Whether it be an issue that arose following your appointment, at the appointment or somewhere in between we would prefer that you let us know than not, so while we are not able to provide service refunds we guarantee that should you be the 0.2% we will resolve any issue you experience so please don’t feel uncomfortable and get in touch.

This applies not only to our Lash Extension service, but across the board for all our services and in regards to any situation you are unhappy. This policy goes further than what is required of us under Australian consumer law and we are happy to offer it!

Protecting Your Health

If this is your first booking at Tan Lash & Beauty we need to ensure you are informed about the services you have booked and any possible contra-indications so you can provide us with informed consent. Like with most fine print, this information covers worst case scenario none of which we can assure you, we have never experienced. Our non-invasive and low risk services are always performed by a specialised, experienced and trained technician holding your safety and care as their number one priority. Your health and well-being is always held paramount when making decision regarding your suitability for a service.

1. General Information for All Services

1. 1 PATCH TESTING

  • I understand that for reasons including;
    • genuine allergic reactions typically take place up to 48 hours post-exposure
    • sensitivities can develop at any time despite laying dormant for years
    • reactions generally will not occur when exposure to small amounts of an allergen in the same way they will when a full procedure is carried out
  • that any  “patch test”  involving one of a range of active ingredients which may be carried out would not necessarily be conclusive or accurate.
  • Despite these points I understand a Patch Test is readily available to me should I wish to undertake one. I understand it is my responsibility to alert and request one from my service provider at my earliest convenience to abide by the bookings timeframe policies.
  • Alternatively, I agree to proceed with full knowledge of the potential risks or any rare allergic reactions which may occur.

1. 2 DISCLOSURE

  • I do not have any known condition, medical or other, which may be aggravated by or affect your ability to safely receive this treatment.
  • I do not have any known allergies to adhesives, synthetics, tape and have not had a previous reaction to this treatment.

1.3 RELEASE

  • ​I understand my technician may request permission to take non-identifying, before and after photos of the procedure. If I consent I grant the use of these images for education and advertising or other purposes.

1.4 LIABILITY WAIVER

  • I understand by proceeding with my appointment I release Perth Tan Lash & Beauty Pty Ltd & Dayiman Pty Ltd from any and all claims, actions, expenses, damages and liabilities, including reasonable legal fees which might be asserted against them as a result of my having this procedure performed, or my purchase of any of their products. As used in this agreement, the term Perth Tan Lash & Beauty Pty Ltd & Dayiman Pty Ltd  include all of their respective, directors, agents, employees, contractors successors and assignees.
  • I understand my consent will remain in effect for the procedure and all future procedures and it is my responsibility to inform my technician of any relevant changes.

2. Eyelash Extensions

2.1 THE PROCEDURE

  • I understand all eyelash extension treatments involve the attachment of individual extensions formulated from synthetic fibres to my own natural eyelashes using medical grade adhesive.
  • I understand the full set of extensions procedure time will be 60 minutes or upwards.  It is my responsibility to keep my eyes closed and be still in a reclined position during the entire procedure. The fumes from the adhesive may cause my eyes to become irritated particularly if I open my eyes before directed.
  • I understand protecting the health, growth and appearance of your natural eyelashes will be held paramount when decisions regarding my suitability for the service are made

2.2 CONTRA INDICATIONS

  • I understand there is a risk eyelash extension adhesive may get underneath contact lenses and cause corneal abrasion or scratching. To avoid this risk contact lenses need to be removed prior to the treatment.
  • I understand in some cases laser eye surgery may cause sensitivity to eyelash extensions or lash lift products. These treatments should be avoided for four weeks after surgery. I will consult my own doctor if this applies to me.
  • I understand medication for chemotherapy may increase the risk of a reaction to this procedure and will consult my doctor if I have had this treatment within the last six months.

2.3 RISKS

  • I acknowledge it is impossible to list all of the potential risks or side affects but some of these are listed below.
  • I understand in rare cases an individual may be allergic or hypersensitive to the materials used causing side effects including but not limited to eye redness, discomfort and irritation.
  • I acknowledge there are rare but potentially harmful or negative side effects  (such as the premature shedding of natural lashes) that the lash procedure or removal may cause to those who have specific medical or skin conditions.
  • I agree that if I experience any of these rare side effects or issues I will contact my technician and if required consult a physician at my own expense.
  • I understand that every care is taken but should an extremely rare allergy or reaction occur this is unforeseeable and no refunds will be issued. With the approval of the clients medical practitioner a free removal will be offered in these circumstances

2.4 CLIENT RESPONSIBILITIES

  • I understand careful aftercare is required to maintain eyelash extensions and these instructions will be outlined. Any follow up care or reduced life of the extensions caused by failure to follow these instructions will be at my own expense and risk.
  • I understand I need to engage with my technician during the initial consultation so clear expectations can be set as all service sales are final and non refundable.
  • I agree to contact the service provider as soon as possible (within 3-4 days) should any issues arise post service so they can work with me to rectify any issues as all service sales are final and non refundable.
  • I understand the cancellation policy as outlined during the booking process and that any changes or cancellations to booked treatments forfeit the required deposit at a minimum regardless of whether or not this was paid at the point of booking.

3. Lash Lift / Brow Lamination

While the the target and results of these two treatments vary the procedure, products, contra-indications and relatively minimal risks associated with non-invasive procedures like these are closely aligned.

3.1 THE PROCEDURE

I understand the Brow Lamination procedure involves the application of products to the eyebrow and its surrounding area to  manipulate the direction of growth towards a desired shape and position performing a semi-permanent re-structure and style. The Brow Lamination package may or may not include shaping via threading, waxing or some trimming as well tinting.

I understand the Lash Lift procedure requires my lashes to be glued to a silicon pad with a water soluble adhesive and lifted onto a silicon pad with a curling agent, a conditioning agent, and nourishing oil. *

3.2 CONTRA INDICATIONS

  • I understand there is a risk any products used may get underneath contact lenses and cause corneal abrasion or scratching. To avoid this risk contact lenses need to be removed prior to the Lash Lift treatment.
  • I understand it is impossible to list every potential risk or contra-indication but some of those known to either prevent or restrict the results include  eye Infections/disorders, recent eye surgery, conjunctivitis, stye, dry eye syndrome, using prescribed medicated eye drops, medication – thyroxin, hormone replacements. If any of these are relevant to me I will consult with a medical practitioner prior to my appointment.

3.3 RISKS

  • I understand in rare cases an individual may be allergic or hypersensitive to the materials used causing side effects including but not limited to skin redness, discomfort and irritation.
  • I acknowledge there are rare but potentially harmful or negative side effects (such as the premature shedding of natural lashes) that the lash procedure or removal may cause to those who have specific medical or skin conditions.
  • I agree that if I experience any of these rare side effects or issues I will contact my technician and if required consult a physician at my own expense.
  • I understand that every care is taken but should an extremely rare allergy or reaction occur this is unforeseeable and no refunds will be issued. With the approval of the clients medical practitioner a free reversal will be offered in these circumstances.

3.4 CLIENT RESPONSIBILITIES

  • I understand I need to engage with my technician during the initial consultation so clear expectations can be set as all service sales are final and non refundable.
  • Should I dislike the results of the procedure no refund for the service is possible but in some cases a free reversal may be offered as a gesture of goodwill.
  • I agree to contact the service provider as soon as possible (within 3-4 days) should any issues arise post service so they can work with me to rectify any issues as all service sales are final and non refundable.
  • I understand the cancellation policy as outlined during the booking process and that any changes or cancellations to booked treatments forfeit the required deposit at a minimum regardless of whether or not this was paid at the point of booking.

4. Henna Brows

  • I understand that Henna Brow Tint involves the application of a semi permanent temporary stain that will fade within a 1-2 week period on the skin and 1-6 weeks on the hairs.
  • I do not have any known condition, medical or other, which may be aggravated by or affect my ability to safely receive this treatment.
  • I do not have any known allergies to henna, henna tattoo products, hair dyes or p-Phenylenediamine (the active ingredient in hair dye).
  • I understand that if I am pregnant or breast-feeding while this does not immediately contra-indicate the procedure it is my responsibility to discuss this with my medical practitioner and make an informed decision on whether to proceed for which I take full responsibility.
  • I understand that for reasons including; that genuine allergic reactions typically take place up to 48 hours post-exposure, sensitivities can develop at any time despite laying dormant for years and that reactions generally will not occur to exposure to small amounts of an allergen in the same way they will when a full procedure is carried out, that any  “patch test”  involving one of a range of active ingredients which may be carried out would not necessarily be conclusive or accurate.
  • However, should I wish to carry out a Patch Test despite this information I understand it is my responsibility to alert and request one from my service provider at my earliest convenience to abide by the bookings timeframe policies.
  • Alternatively, I agree to proceed with full knowledge of the potential risks or any rare allergic reactions which may occur.
  • I agree that if I experience any rare allergic reaction or side affect I will contact my technician and if required consult a physician at my own expense.
  • I understand that Henna may not stain any scars I have within the area the Henna is applied.
  • I Understand that although Henna is a low risk treatment I could have an allergic reaction.
  • I understand if I have any reaction to Henna, I will see my health care provider for further treatment at my own expense.
  • I understand this treatment can not safely be performed if there are any cuts or abrasions/open wounds in the area of application. It is my responsibility to ensure there are not.
  • I understand the consumption of Oral Roaccutane medication is a contraindication for this procedure and it is my responsibility to disclose this information and exclude myself.
  • I understand by proceeding with my appointment I release Perth Tan Lash & Beauty Pty Ltd from any and all claims, actions, expenses, damages and liabilities, including reasonable legal fees which might be asserted against them as a result of my having this procedure performed, or my purchase of any of their products. As used in this agreement, the term Perth Tan Lash & Beauty Pty Ltd include all of their respective, directors, agents, employees, contractors successors and assigns
  • My consent will remain in effect for the procedure and all future procedures and it is my responsibility to inform my technician of any relevant changes.

5. Waxing

  • I understand the Henna Brow make-over does include an option for a Eyebrow Reshape which can include waxing.
  • Waxing is a method of temporary hair removal which removes the hair from the root. Waxing is relatively safe and a low-risk treatment.
  • I do not have any known condition, medical or other, which may be aggravated by or affect my ability to safely receive this treatment.
  • I do not have any known allergies to any type of wax and have not had a reaction in the past to this procedure.
  • I understand that for reasons including; that genuine allergic reactions typically take place up to 48 hours post-exposure, sensitivities can develop at any time despite laying dormant for years and that reactions generally will not occur to exposure to small amounts of an allergen in the same way they will when a full procedure is carried out, that any  “patch test”  involving one of a range of active ingredients which may be carried out would not necessarily be conclusive or accurate.
  • However, should I wish to carry out a Patch Test despite this information I understand it is my responsibility to alert and request one from my service provider at my earliest convenience to abide by the bookings timeframe policies.
  • Alternatively, I agree to proceed with full knowledge of the potential risks or any rare allergic reactions which may occur.
  • I am aware that discomfort may occur.
  • I am aware that waxing may have some side effects including, but not limited to, redness, scabbing, bruising, scarring, swelling, tenderness, hyper-pigmentation, flaking and/or pimples.
  • I agree that if I experience any rare allergic reaction or side affect I will contact my technician and if required consult a physician at my own expense.
  •  I agree to apply a sunblock with appropriate level of SPF to any newly waxed areas exposed to sunlight.
  • I agree to avoid the following after the waxing procedure: saunas, steam rooms, hot tubs or other heat sources; avoid application of Retin-A, Renova or AHA products for 48 hours; avoid using a loofah or other abrasives on the waxed area.
  • I understand the consumption of Oral Roaccutane medication is a contraindication for this procedure and it is my responsibility to disclose this information and exclude myself.
  • I understand by proceeding with my appointment I release Perth Tan Lash & Beauty Pty Ltd from any and all claims, actions, expenses, damages and liabilities, including reasonable legal fees which might be asserted against them as a result of my having this procedure performed, or my purchase of any of their products. As used in this agreement, the term Perth Tan Lash & Beauty Pty Ltd include all of their respective, directors, agents, employees, contractors successors and assigns
  • My consent will remain in effect for the procedure and all future procedures and it is my responsibility to inform my technician of any relevant changes.

6. Spray Tanning

  • I do not have any known condition, medical or other, which may be aggravated by or affect my ability to safely receive this treatment.
  • I do not have any known allergies to the ingredients in spray tan products and have not had a previous reaction to this treatment.I understand the active ingredient in all Spray Tans is Dihydroxyacetone (DHA) which is a sugar cane derivative that has been scientifically proven to be an effective tanning agent . It was approved by the FDA in 1973 and is considered “safe” as it only stays on the top layer of the skin and is not absorbed by the body.
  • I understand this product does not contain a sunscreen and does not protect against sunburn. Repeated exposure of unprotected skin may increase the risk of skin aging, skin cancer and other harmful effects to the skin even if you do not burn.
  • I understand that the following conditions may affect my ability to safely receive this service; severe asthma or contagious skin conditions. If I am affected by any of these I will seek medical advice to determine my ability to safely receive it prior to the appointment. The following conditions may affect the results of the service and I acknowledge these results are out of the control of the service provider; pigmentation disorders, sunburn, psoriasis, eczema.
  • I understand no conclusive research has been done regarding the safety of spray tanning during pregnancy. While the service provider does provide this service to pregnant customers and has extensive extraction equipment to reduce the risk of overspray inhalation it is up to me to seek medical advice prior to the service and I understand I assume all risks should I proceed.
  • I understand I need to engage with my technician during the initial consultation so clear expectations can be set as all service sales are final and non refundable.
  • I understand that for reasons including; that genuine allergic reactions typically take place up to 48 hours post-exposure, sensitivities can develop at any time despite laying dormant for years and that reactions generally will not occur to exposure to small amounts of an allergen in the same way they will when a full procedure is carried out, that any  “patch test”  involving one of a range of active ingredients which may be carried out would not necessarily be conclusive or accurate.

  • My consent will remain in effect for the procedure and all future procedures and it is my responsibility to inform my technician of any relevant changes.

 7. Hair Styling and Makeup Application

  • I do not have any known condition, medical or other, which may be aggrevated by or affect your ability to safely receive this treament.
  • I do not have or have disclosed any known allergies to cosmetic or hair products and have not had a previous reaction to this treatment.
  • I understand the GLAM service can include the direct topical application of cosmetic products by a therapist and that hair styling may include the use of products and electrical tools which use heat.
  • I understand that for reasons including; that genuine allergic reactions typically take place up to 48 hours post-exposure, sensitivities can develop at any time despite laying dormant for years and that reactions generally will not occur to exposure to small amounts of an allergen in the same way they will when a full procedure is carried out, that any  “patch test”  involving one of a range of active ingredients which may be carried out would not necessarily be conclusive or accurate.
  • However, should I wish to carry out a Patch Test despite this information I understand it is my responsibility to alert and request one from my service provider at my earliest convenience to abide by the bookings timeframe policies.
  • Alternatively, I agree to proceed with full knowledge of the potential risks or any rare allergic reactions which may occur.
  • I agree to release the service provider from liability for any skin complications due to allergic reactions or other and from any damage caused to hair or other injury caused by styling products or tools.
  • I understand I need to engage with my technician during the initial consultation so clear expectations can be set as all service sales are final and non refundable.
  • I agree to contact the service provider as soon as possible (within 3-4 days) should any issues arise post service so they can work with me to rectify any issues as all service sales are final and non refundable.
  • I understand by proceeding with my appointment I release GLAM and Perth Tan Lash & Beauty Pty Ltd from any and all claims, actions, expenses, damages and liabilities, including reasonable legal fees which might be asserted against them as a result of my having this procedure performed, or my purchase of any of their products. As used in this agreement, the term Perth Tan Lash & Beauty Pty Ltd include all of their respective, directors, agents, employees, contractors successors and assigns
  • My consent will remain in effect for the procedure and all future procedures and it is my responsibility to inform my technician of any relevant changes.
